
Scenario 1
If you already hold a U.S. passport, do not have a Chinese passport, and are applying for a Chinese visa for the first time, you will need a "Birth Certificate" issued by a Chinese notary office.
Scenario 2
If you are living in the United States and have lost your Chinese passport, requiring a replacement, you will also need a "Birth Certificate" issued by a Chinese notary office.
Generally Required Documents:
1) Scanned copies of personal ID and parents' IDs;
2) Scanned copy of household registration booklet;
3) Preferably a local kinship certificate;
4) Digital version of passport-sized photo.
