Why Do You Need to Process the DS3053 Form?
If the father is not in the U.S., two notarizations are required:
- DS3053 notarization for applying for a U.S. passport;
- Travel document authorization notarization for applying for a travel permit.
Generally Required Documents:
- Child's hospital birth certificate;
- Passport first page of both parents (the green card holder must provide their ID card).
