Documents Required for Chinese Birth Certificate Notarization
- Scanned copies of both sides of the applicant's and parents' ID cards, household register (cover page, household head page, and personal information pages of all three individuals), and parents' marriage certificate/divorce certificate;
- Birth certificate/one-child certificate (one of these). If unavailable, a certificate issued by the local police station is required (must be printed and stamped, not handwritten, and include the station's landline number and the name of the issuing officer for verification by the notary office);
- For use in the U.S., a digital copy of a one-inch white-background photo is required;
Note: All documents must be provided as color scanned digital copies, sized 1:1 with the originals.
