Required Documents for Notarization of Power of Attorney/Consent Form:

1. Two copies of the signed Power of Attorney (do not fill in the date initially) to be mailed, and a video recording of no less than 10 seconds during the signing process must be printed and sent to the customer service email.
2. Scanned electronic copies of both sides of the ID card/passport of the principal and the agent, and household register (cover page + householder's personal information page + applicant's page + related person's page).
3. A photo of the principal holding their ID card and the signed Power of Attorney (taken indoors against a white wall background).
4. Electronic scanned copies of parents' marriage certificate, minor's birth medical certificate, and passport.

Note: All documents must be provided as color scanned electronic copies, sized 1:1 with the originals.
