Required Materials for Notarization of Power of Attorney/Consent Form:

1. Two copies of the power of attorney signed without the date, mailed in advance, and a video recording of no less than 10 seconds during the signing process printed and sent to the customer service email.
2. Scanned electronic copies of the front and back of the ID cards/passports of the principal and the agent, and the household register (first page + householder's personal information page + applicant's page + related person's page).
3. A photo of the principal holding their ID card and the signed power of attorney (taken indoors against a white wall background).
4. Scanned electronic copies of the parents' marriage certificate, the minor's birth medical certificate, and passport.

Note: All documents must be provided as color scanned electronic copies, sized 1:1 with the originals.
