Required Materials for Notarization of Power of Attorney/Consent Form:
1. Two copies of the signed Power of Attorney (do not fill in the date), mailed along with a printed version. Record a video of no less than 10 seconds while signing and send it to the customer service email.
2. Scanned electronic copies of both sides of the ID cards/passports of the principal and the agent, and household register (cover page + householder's personal information page + applicant's page + related person's page).
3. A photo of the principal holding their ID card and the signed Power of Attorney (taken indoors against a white wall background).
4. Scanned electronic copies of parents' marriage certificate, minor's birth medical certificate, and passport.
Note: All documents must be provided as color scanned electronic copies, sized 1:1 with the originals.
