Section 3 – Content for Notarization/Authentication Reference:

• If no option matches, check "Other" and enter the actual content, e.g., Birth Certificate Authentication, Statement, Power of Attorney for Child Foster Care, etc.

Section 4 – Purpose of Notarization/Authentication Content Reference:

If no option matches, check "Other" and fill in with reference to the following:

• For school enrollment, company registration, authorizing real estate transactions, household registration in China, settling and registering residence when returning to China with parents;
• For work, study, or residence in China;
• For processing residence procedures, children's school enrollment procedures, etc.

Note on Document Usage Location: Specify the province/city, e.g., Beijing, China; Hunan, China.

Section 5 – Submitted Materials and Number of Notarization/Authentication Applications:

• Enter "Authentication document and passport copy" or "Driver's license copy," etc.
• Section 5 – Quantity: Enter the actual number, i.e., the number of agency services purchased.

Tip: Main document name + passport copy, e.g., Birth Certificate, Passport Copy.

Other optional content references:

• For Birth Certificate Authentication, provide: Original birth certificate, child's passport copy, parents' passport copies, and copies of U.S. status documents;
• Original and copy of the birth certificate authenticated by the Secretary of State;
• For Property Sale Power of Attorney: Power of Attorney, principal's passport/ID copy, agent's ID copy, property deed copy, etc.

Section 6 – Processing Time: If opting for expedited service, additional fees apply. If you have not paid for expedited service, please select regular processing!

If you are processing documents like a Power of Attorney, ensure the signature matches exactly as on the Power of Attorney. For example, if the Power of Attorney is signed in Chinese, sign this form in Chinese; if signed in both Chinese and English, sign accordingly on the form.
