One Application Form — The "Application Form for Passport, Travel Document, or Return Permit of the People's Republic of China," completed and printed after online appointment and form filling.

Two Standard Photos — Four recent 2-inch, front-facing, bareheaded, glossy, white-background paper photos taken within the last six months. One of these should be attached to the application form.

Photo CD — Digital photos are used for e-passports. It is strongly recommended to also provide a CD or DVD with the same digital photo version (external hard drives or other portable storage media are not accepted).

Copy of Original Passport — (If available) or a copy of photo identification (if available). If no passport copy is available, provide a copy of one of the following documents proving Chinese nationality and identity: household register, ID card, or birth certificate, etc.

Original and Copy of Legal Residence Proof in the U.S. and Other Countries — Such as Green Card, work or study visa, I-797 form, I-20 form, etc.
