What Documents Are Required for a Chinese Travel Document?

1. One application form: The "Application Form for Passport, Travel Document, or Return Permit of the People's Republic of China," completed online, printed, and scheduled for an appointment.
2. Two standard photos: Two recent 2-inch, front-facing, bareheaded, glossy, white-background paper photos taken within the last six months. One should be attached to the application form.
3. Original and copy of the child's birth certificate: Original and copies of both parents' valid passports, including the photo page, any pages with annotations or extensions, U.S. visa pages, the most recent Chinese exit stamp, and the U.S. entry stamp.
4. Original and copy of the parents' marriage certificate (if applicable).
