What Documents Are Required for a Chinese Travel Document?
- One application form: The "Application Form for Passport, Travel Document, or Return Permit of the People's Republic of China," completed online, printed, and scheduled for an appointment.
- Two standard photos: Two recent 2-inch, front-facing, bareheaded, glossy, white-background paper photos taken within the last six months. One should be attached to the application form.
- Original and copy of the child's birth certificate: Original and copies of both parents' valid passports, including the photo page, any pages with annotations or extensions, U.S. visa pages, the most recent Chinese exit stamp, and the U.S. entry stamp.
- Original and copy of the parents' marriage certificate (if applicable).
- Original and copy of the parents' residence permits: Original and copies of both parents' residence permits in the U.S. or other countries at the time of the child's birth and currently (e.g., work or study visas, I-797 form, I-20 form, etc.).
- If one parent cannot accompany the child:
1. Submit a copy of their valid passport (entire passport, including blank pages);
2. Provide the original notarized consent letter (click to download sample consent letter) signed by the absent parent and notarized by a local notary office (if in China, it must be notarized by a Chinese notary office), authorizing the spouse to apply for the travel document for the child to travel to China. - Original and copy of the previous travel document or other certificates: If the child has previously held a travel document, submit the original and a copy. If the child has traveled to China and returned to the U.S. using a travel document, also submit the original and copies of other documents used for entry and exit (e.g., entry-exit permit, U.S. passport, etc.), including the photo page, Chinese entry stamp, Chinese exit stamp, and U.S. entry stamp pages.

If the passport or travel document applicant is under 16 years old, their parents or other legal guardians must accompany them. Details are as follows:

1. If the guardians are the applicant's parents, both parents must be present, sign the relevant sections of the application form, and provide original and copies of their valid identification documents (e.g., passport, driver's license, green card, or ID card) and proof of relationship (e.g., notarized birth certificate, notarized kinship certificate, birth medical certificate, or relevant pages of the household register).
2. If one parent cannot be present, the absent parent must provide a copy of their valid identification document and the original and copy of a notarized consent letter authorizing the other parent to apply for the passport or travel document on behalf of the child.
3. If the guardian is not the applicant's parent, the guardian must accompany the applicant, sign the relevant sections of the application form, and provide original and copies of their valid identification document (e.g., passport, driver's license, green card, or ID card) and legal documents issued by U.S. authorities or the applicant's parents proving their guardianship rights.
